BCIwiki (bciwiki.com) — A review proposes closed-loop brain-computer interfaces (BCIs) as a way to partially restore the disrupted sensorimotor loop after stroke, detecting motor imagery, motor attempt or sensorimotor rhythms in real time and translating them into functional electrical stimulation, robotic assistance, virtual reality, multisensory feedback or neuromodulation. The study was published in Frontiers in Neuroscience on August 20, 2026.
The authors synthesize the neurobiological basis, recovery mechanisms, implementation strategies and emerging predictive biomarkers of closed-loop BCIs for post-stroke sensorimotor rehabilitation. They note the systems show promise for selected patients, but treatment effects remain heterogeneous, depending on patient characteristics, decodability of motor-related brain signals, intervention dose, feedback modality and trial design; future progress will require mechanism-informed patient stratification, adaptive decoding, individualized feedback, standardized outcomes and real-world validation.
